• Cremation Authorization
  • This is one of several legal requirements needed before the cremation can take place.  This form authorizes the funeral home and crematory to perform the cremation after the death certificate has been signed and burial/cremation permit has been issued.
• Pre-Need Authorization
  • This form is basically the same as above except the form is filled out by the person requesting the service.
• Ante-mortem Authorization
  • Same as the pre-need form.
• Receipt for Decedent
  • This is the receipt forms used between the funeral home and crematory receiving the decedent (dead body).
• Receipt for cremated remains
  • The receipt given to whoever picks up or receives the cremated remains of the decedent after the cremation process.
• Transfer of the Decedent
  • Authorization to transfer the deceased person.
• Appointment of Agent
  • Forms used to appoint the agent responsible to authorize the funeral home and/or crematory the go ahead to perform the cremation.
• Alternate Agent Appointment
  • Form used if next-of-kin is not available.
• Irrevocable Assignment
  • Assignment of insurance proceeds used to pay the funeral/cremation costs.
• Next of Kin Appointment
  • The form establishing the person (party) with the primary right of disposition, when more than one person has the primary right of disposition.
• Wavier
  • Wavier signed by person authorizing the cremation when they are not next-of-kin and know that there are not any next-of-kin available to authorize the cremation.
• Question form for booklet
  • This form is provided to use when asking question to be answered in "Funerals: Every thing you want to know but too afraid to ask."  The title referred to is a booklet I am currently working on.
